188 Calcific versus non-calcific plaque: a cad-rads and ffrct study

Background Coronary Artery Disease-Reporting and Data System (CAD-RADS) standardises Computed Tomography Angiography (CTCA) reporting. calcification can overestimate stenosis. We hypothesized where CAD-RADS category is assigned due to predominantly calcified maximal stenosis (Ca+), the CTCA-derived Fractional Flow Reserve (FFRCT) would be lower compared non-calcified stenoses (Ca-) of same category. Methods Consecutive patients undergoing routine clinical CTCA (September 2018 May 2020) with ≥1 ≥25% FFRCT correlation were included. CTCAs subdivided into Ca+ Ca-. was measured in left anterior descending (LAD), circumflex (LCx) right coronary artery (RCA). Potentially flow-limiting classified as FFRCT≤0.8. A subset had Invasive (ICA). Results 561 screened, 320 included (60% men, 69±10 years). 51%, 69% 50% 2, 3 4 respectively. There no difference prevalence FFRCT≤0.8 between Ca- for each categories. No demonstrated median or end-vessel within 2 4. a (maximal p= .02, .005) vs obstructive disease at ICA any Conclusion values rate both suggestive an underestimation based on but not corroborated ICA.
